Support plan
With support from a parent or carer, the young person identifies how they would like to live their life. This includes specific goals or outcomes they would like to achieve. The young person and carer's outcomes will be related to the young person's personal goals and how they want to live their life.
To decide on individual goals the young person in partnership with their parent and, or carer might consider:
- What works and doesn't work in their life at the moment
- What is important to them and what they want to do
- What support do they and their carer need and want
- What ideas they have about how to make these things happen
- How to keep safe
- How to make decisions relating to their life
The person who helps you to prepare your support plan will help you to consider the risks involved and how these may be managed.
The parent and, or carer and the child or young person meet with a worker at least every twelve months to update the plan. They will need to know how your plan is working and if the outcomes are being achieved.
